Key Responsibilities Serve as the SNBC (Special Needs Basic Care) Coordinator, ensuring access to healthcare services, providing advocacy, and helping clients navigate complex healthcare delivery systems. Conduct comprehensive nursing assessments in assigned programs, providing information and referrals to health and human services programs through home visits and community settings. Provide ongoing case management for clients, maintaining case notes and client files to meet State and Federal requirements. Deliver health teaching and education to promote positive health outcomes and prevent illness, investigating disease and other health events as needed. Foster collaborative relationships with the public, community organizations, and other agencies to promote health and well-being. Respond to disasters and assist communities in recovery, recognizing deviations from the norm that might indicate an emergency. Essential Qualifications To be successful in this role, you will need: A current Registered Nurse (RN) licensure in the state of Minnesota. A thorough understanding of public health nursing theory and practices, with the ability to apply principles to communities, families, and individuals. Excellent communication and interpersonal skills, with the ability to establish and maintain effective relationships with clients, agency staff, and other stakeholders. The ability to work independently, making informed decisions and utilizing community resources effectively. A valid driver's license, with the ability to travel to various community settings. Preferred Qualifications While not required, the following qualifications are highly preferred: A Bachelor of Science in Nursing (BSN) degree or Public Health Nursing (PHN) certification. A minimum of 1-2 years of experience in public health nursing or a related field.
