WellSpan Health is seeking a dedicated and compassionate Registered Nurse (RN) – Virtual Patient Care to join our innovative remote healthcare team. Responsibilities:
As a Virtual RN, you will:
- Conduct remote patient assessments via phone or video.
- Monitor and manage patients with chronic conditions and post-hospitalization needs.
- Educate patients and families on care plans, medications, and self-management.
- Collaborate with physicians, specialists, and care teams for seamless care coordination.
- Utilize telehealth technology to document and track patient progress.
- Triage patient symptoms and provide guidance based on clinical protocols.
- Address patient concerns, escalating issues when necessary.
- Ensure compliance with HIPAA, patient safety, and regulatory guidelines.
- Participate in virtual team meetings and continuous professional development.

Requirements:
To be successful in this role, you should have:
- A valid RN license in [State(s) required].
- Minimum of two years of clinical experience, preferably in telehealth, primary care, or case management.

- Proficiency in electronic health records (EHR) and telehealth platforms.
- Experience with triage protocols and virtual patient assessments.
- Reliable internet connection and a dedicated workspace.

- Flexibility to work shifts, including evenings or weekends if needed.
