Key Responsibilities: Coordinate and manage patient transitions from hospital to home or other care settings, ensuring continuity of care and reducing readmission rates. Collaborate with healthcare providers, patients, and families to develop personalized care plans that meet unique needs and goals. Educate patients and caregivers on self-care practices, medication management, and follow-up appointments to promote optimal health outcomes. Monitor and evaluate patient progress, making adjustments to care plans as necessary to ensure effective care coordination. Ensure timely follow-up with patients and primary care providers to address any issues or concerns, providing ongoing support and guidance throughout the transition process. Document all patient interactions, assessments, and care plans in accordance with hospital policies and procedures, maintaining accurate and up-to-date records. Participate in interdisciplinary team meetings to discuss patient progress and discharge planning, contributing to a collaborative and patient-centered approach to care. Serve as a resource for patients and families, providing emotional support, guidance, and education to empower informed decision-making and self-advocacy. Qualifications: Current RN license in FL or compact license that allows work in FL, with a minimum of 2 years of experience in a clinical or hospital setting. Preferred experience in transitional care management or case management, with strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams. Strong organizational skills and attention to detail, with the ability to manage multiple priorities in a fast-paced environment.
