About the position Responsibilities • Provide support to complex members for care coordination and case management services. • Engage with members, caregivers, and providers to assess and plan individual member goals. • Complete comprehensive person-centered assessments of physical, psycho-social, and environmental health. • Identify problems/barriers for care coordination and implement appropriate interventions. • Coordinate physical, behavioral health, and social services for members. • Manage medications, including regular reconciliation and adherence support. • Create and implement a plan of care to assist members in achieving health goals. • Identify short- and long-term goals with associated time frames for completion. • Document progress towards meeting goals and resolving issues. • Schedule follow-up calls and make appropriate referrals. Requirements • Current and unrestricted Registered Nurse licensure in Ohio or licensed Social Worker (LSW or LISW) in Ohio. • Bachelor's Degree (or higher) in a health-related field preferred. • Minimum of 3 years of clinical practice experience with Medicaid populations. • Minimum of 3 years of case management experience preferred in behavioral health. • Proficiency in MS Office (Word, Excel, Outlook, Teams) and electronic medical record programs. • Valid driver's license, car insurance, and reliable vehicle.
