Key Responsibilities
As an RN Case Manager, you will play a vital role in ensuring our members receive accessible and appropriate care. Your primary responsibilities will include:
- Managing a dedicated caseload within one borough (Bronx, Brooklyn, or Manhattan), ensuring members receive comprehensive care and support.
- Developing, overseeing, and updating individualized care plans to align with members' evolving needs and goals.
- Conducting home visits (4-6 visits per week) to assess home safety and care needs, identify potential risks, and develop strategies to mitigate them.
- Collaborating with leadership and Intensive Care Management teams to address complex cases, share best practices, and drive quality improvement initiatives.
- Presenting high-risk cases for multidisciplinary review, contributing to data-driven decision-making and care planning.
- Maintaining accurate and up-to-date records, ensuring compliance with regulatory requirements and organizational policies.
Essential and Preferred Qualifications
To be successful in this role, you will need:
- Active New York State RN License required.
- Bachelor's degree in Nursing, Health, or Human Services preferred.
- Minimum two (2) years of home care experience required, with a focus on care planning, patient assessments, and home safety evaluations.
- Strong experience in care planning, patient assessments, and home safety evaluations essential.
- Prior supervisory experience a plus.
- Bilingual candidates strongly preferred (see language preferences by borough below):
- Bronx: Bilingual Spanish preferred.
- Brooklyn: Bilingual Chinese, Spanish, or Russian preferred.
- Manhattan: Bilingual Cantonese or Mandarin preferred.
- Proficiency in Microsoft Office required.
