Key Responsibilities Patient Assessments and Care Planning: Performs comprehensive patient assessments and collaborates with the care team to develop and implement a plan of care that meets the patient's specific needs. Referrals and Coordination: Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so. Patient Education and Support: Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques. Supervision and Leadership: Supervises LPNs and HHAs, providing guidance and support as needed. Documentation and Communication: Completes documentation timely and accurately, and regularly communicates patient progress to the clinical manager and care team. On-Call Responsibilities: Performs on-call responsibilities and on-call services to patients/families as assigned. Professional Development: Participates in clinical development and continuing education programs to stay up-to-date on best practices and industry developments. Essential Qualifications RN License: Current RN license, specific to the state(s) you are assigned to work. Clinical Experience: One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical. CPR Certification: Current CPR certification. Valid Driver's License: Valid driver's license, reliable transportation, and liability insurance. Preferred Qualifications Home Health Experience: Previous experience in home health or hospice care. Leadership Experience: Experience supervising LPNs and HHAs. Skills and Competencies Clinical Skills: Strong clinical skills, including assessment, planning, and implementation of patient care. Communication Skills: Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare professionals. Time Management: Ability to manage time effectively, prioritizing tasks and managing multiple patient visits. Compassion and Empathy: Compassionate and empathetic, with a patient-centered approach to care. What We Offer Amedisys offers a competitive salary range of $75,000 to $88,000, depending on location, job-related knowledge, skills, and experience.
