About the Role: Diabetes Educator - Registered Nurse - RN - Case Management We are seeking an experienced and dedicated Diabetes Educator and Registered Nurse - RN to join our team as a Case Management Specialist. This is a full-time, remote position that offers the opportunity to work with a vibrant and diverse team, making a meaningful difference in the lives of our members. As a Case Manager RN, you will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate overall wellness. Your expertise in diabetes education and management will be crucial in developing proactive courses of action to address the complex health and social challenges faced by our members. Key Responsibilities: Conduct comprehensive assessments of members' needs and benefits, utilizing clinical tools and information/data review to identify risk factors and barriers to care. Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers, addressing complex health and social indicators that impact care planning. Collaborate with supervisors and other key stakeholders in the member's healthcare to overcome barriers in meeting goals and objectives. Utilize case management processes in compliance with regulatory and company policies and procedures, ensuring maximum member engagement and discerning their health status and health needs through motivational interviewing skills. Work on behalf of the client and the member's provider, advocating for their needs and facilitating access to necessary resources and services. Essential Qualifications To be successful in this role, you will need to possess the following essential qualifications: Be a Registered Nurse with an unrestricted nursing license and multi-state/compact privileges, with the ability to be licensed in all non-compact states. Have a minimum of 3 years of clinical/nursing practice experience, with 3+ years of diabetes management experience and 1+ year of experience in care coordination or discharge planning. High-speed internet access and the ability to hardwire your computer, with proficiency in personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications. Reside in the Continental United States. Preferred Qualifications While not required, the following preferred qualifications will be considered an asset: Telephonic case management experience. Excellent communication and interpersonal skills. Education: Associate's degree in nursing required, with a BSN preferred.
